Treatment Of Consent (Under 16’s)

Most parents are likely to bring their child to the surgery themselves. However, occasionally it is necessary for another adult to bring a child for treatment, for example, immunisations. It is a legal requirement for the surgery to have the consent of a parent or legal guardian before this can be done.

The forms are available from Reception and a separate form is required forĀ EACH VISIT. Please feel free to take a couple just in case;
