Sickness Certificates

You do not require a doctor’s sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.

If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. If your employer insists on a Doctor’s note regardless, this will involve a charge.

Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.

If you have been ill for more than 7 days you will need a doctor’s certificate. These certificates are called ‘Fitness To Work’ Certificates.

The doctor may call you or arrange for an appointment.

If you have already been seen by your Doctor about this illness your certificate will be sent to you within a week.

Statement of Fitness for Work – Sick Note

Patient’s would normally apply for a “Sick Note” by booking an appointment with their doctor, however due to the Coronavirus outbreak, the practice is unable to issue “sick Notes” to COVID-19 patient’s and for those patient’s who have bee advised to self-isolate. Patient’s who have been advises to self-isolate for COVID-19 will soon be able to obtain an alternative to the Sick Note” to cover this by contacting NHS 111, rather than booking an appointment with a doctor.

Requesting a Sick Note

If you have already had a Sick Note (Fit Note) for this illness your Doctor may not need to see you to issue an additional Sick Note. Please send an email to to the practice with the following information;

  • Your name
  • Date of birth
  • Contact number on file
  • Your email address
  • The first date you were not at work due to the illness
  • The total number of days that you were ill or if the illness is still ongoing
  • The reason for why you need a sick note

We may contact you to arrange an appointment. Alternatively, your certificate will be sent to you within a week.

Patient’s are only seen through a triaged process. any patient’s that are exhibiting COVID-19 symptoms (coughs, shortness of breath, fever) will not be seen by a GP.
